FOB Assignment Process

Please note that the FOB Process Guide previously distributed is incorrect. We apologize for any inconvenience. Please review the instructions below on how to assign a FOB to a member: 

 

FOB Assignment Process:

  1. The Case Manager (CM)/Support Coordinator (SC)/Targeted Case Manager (TCM) will install the FOB with a security tie in the member's home. 

  2.  The CM/SC/TCM will submit a linking form to HHA. 

  3. Once submitted, a request will be routed directly to HHAX Support to activate the device. 
  4. The device will be activated within one (1) business day of the request being submitted.
