FOB Assignment Process:

The Case Manager (CM)/Support Coordinator (SC)/Targeted Case Manager (TCM) will install the FOB with a security tie in the member's home.

The CM/SC/TCM will submit a linking form to HHA.
- Once submitted, a request will be routed directly to HHAX Support to activate the device.
- The device will be activated within one (1) business day of the request being submitted.
